T.J., a 30-year-old African American client, is in his last year of law school and is clerking for a prestigious law firm. He and his fiancé plan to marry as soon as he graduates. During the last week he has had four dizzy spells and a headache at the base of his skull upon awakening for the last 2 days. His father has a history of hypertension, so T.J. is aware that his symptoms may indicate high blood pressure. On his way home from work, T.J. stops by the clinic and asks the nurse to check his blood pressure. The nursing assessment yields the following data.

Subjective data: States he has had four dizzy spells and has awakened with a headache in the occipital lobe the last two mornings. T.J. has 1 glass of wine at lunch and 2-3 beers in the evening to relax from the tension of school and work. Most of his meals are at fast-food establishments and have a high fat content. T.J. does not smoke. He used to jog 4 mornings a week but quit when he started clerking. He has had nocturia for the last 3 weeks. He is not taking any medication. T.J. states he is concerned about having hypertension because he does not want to take medication.

As you reflect on this story, how does it help you understand that pain is a subjective experience for your client?

Aron Ralston, an avid outdoorsman, was hiking in a narrow canyon in Southern Utah when he upset a large boulder that rolled down the canyon after him. As it fell, the boulder relodged itself between the narrow canyon walls and in the process pinned Aron’s right wrist between the boulder and the canyon wall. Aron was alone and was unable to free himself from the situation. After being trapped for five days, with no help arriving, Aron eventually made the decision to free himself by breaking the bones in his forearm with the weight of his body and amputating his hand with a dull pocketknife. After freeing himself, he hiked an additional seven miles before the search and rescue crews found him. His arm required further surgery to repair it. After the surgery, the nurses would ask Aron, “On a scale of 0-10, what is your pain level with 0 being no pain and 10 being the most pain you have ever experienced?”. Aron would reply, “0.25”. When asked about giving such an odd answer, he stated that even though he was in pain after the surgery, it was minimal compared to the pain he experienced when he cut through his ulnar nerve to free himself from being trapped by the boulder.

Background Information:
When studying the different religions of the world we tend of focus on the distinctions between these religions and understanding what makes them unique, however it is important to note that although these religions are distinct and unique in their own way, there are common themes and structures that can be applied to them. Some of the common elements present in the different religions that we will be studying this semester are:

Sacred Scriptures and Sacred Writings

· Divinely inspired writings that contain origins of the religion, essential beliefs, and major teachings. Some religions also revere writings of prominent figures throughout their history who have contributed to expounding the theology of the religion (ex: writings of the Christian saints)

Beliefs and Practices

· Tenets of the religion that unify its community of believers on a variety of topics ranging from their relationship with the divine, soteriology, creation, eschatology, anthropology, etc. 

·
Prayer and Worship

· Liturgical practices that believers of the faith participate in throughout their daily lives. Some liturgical practices and rituals of prayer are performed in conjunction with the celebration of religious festivals at particular times of the week (ex: Sabbath) or times of the year (ex: Christmas)

Morality – Code of Ethics

· Guiding principles that adherents of the faith abide by throughout their lives informed by the theology of the religion

History

· Understanding how the religion was established and the advances of the religion throughout human history that shape the global image of the religion today while also recognizing changes in theology that have occurred throughout history due to major historical events

Throughout his life, Siddhartha Gautama did not record any of his teachings, but he is known for his influential sermons and the discussions that he had with his fellow monks. These sermons were transmitted orally for centuries before they were finally committed to writing by several different Buddhist groups. One of the most famous of 
The Buddha’s discourses is his first discourse titled 
Setting in Motion the Wheel of Dhamma and in this discourse he outlines two ideas that would become the foundation of all Buddhist thought: 

The Four Noble Truths
 and 

The Noble Eightfold Path.
